Default Tearing issues

For the most part, any tearing I experience can be remedied by a quick trip to the graphic options section of whatever game I may be running. However, I find that every now and then (probably once ever 10 minutes or so) I get an incredibly large burst of what I assume is tearing, and only for a split second. It's almost as though the right half of my screen shreds through my left side as if it were dislocating itself and popping itself back in. It doesn't leave any sort of blackness or emptiness in its wake, and since it's very quick it's very hard to describe.

Setup details that I can think of off the top of my head:

nVidia GTX550Ti (always running at 1920X1080 if the game allows, otherwise maximum allowed)
Monitor is a 21" LED TV with VGA input. Runs at 60hz.

The "tearing" only seems to happen when playing a game, so I don't know if it's related to my refresh rate, my drivers, or even just the games in general.

I'm going to try to get some kind of video to capture it, since it's difficult to put into words.

Generally, any teraing I do get I only expect to be a couple of lines, and almost always in the top 1/4 of my display. The massive tears I've been having are maybe more accurately described as "shifts." Rather than a horizontal misplacement due to a drawing issue, this is more of a break along a vertical axis.

Gonna do the best I can to snap a video or something of it.

Any tearing will be fixed if vsync is set to on, however this has been known to introduce mouse lag. Usually what I will do is run a specific game with Fraps showing the frame rates and then I will increase the FSAA and AF levels until I can get the average frame rates to close to the refresh rate of my screen (usually 60fps), this prevents tearing without introducing mouse lag.
