Not sure about the bsod code, but looks to me like you need more vcore. I have an Asus board and vccsa/vccio voltages are no where near that high.

Give us information about your DRAM timings and speed. It could be your DRAM limiting your o/c... I highly suggest keeping your voltages MUCH lower than X58 as SNB is weaker to high voltages (You will kill your chip faster with voltages close to normal X58 o/c).

Stay below 1.35v vcore for sure, and VTT I am not so sure, but I'd say to try and stay below 1.4v

I clocked a 2500k last night to 4.7GHz (under 70c with a TT Frio, and LinX 20 passes stable at Max. memory) only raising the Vcore to 1.355V, nothing else. I think too that your Vcore is low for 4.5.

I'm stable at 4.5GHz on my 2600k at around 1.24V, I've set it to offset + mode and this is what it hits at load.
I've stress tested for 4 hrs on Prime95, I feel that's sufficient enough to have a stable OC.

I clocked a 2600k to 4.5ghz by doing nothing but setting DRAM voltage to 1.65 and CPU voltage to 1.30

Turned on low vdroop.
