An update:
RMA was fast, I got the new drive almost the next day in Toronto, BUT the second drive failed after a month, constant system freezes, BSODs, just like what other people wrote in the feedback. I think OCZ should just send out newer models for replacement
Formated the drive, it worked for a few days followed by a BSOD and bad sectors again:
