I find I typically suffer eyestrain, headaches, etc. after a few hours of use if the refresh rate is below 75 Hz. Generally, the higher the better, though it wears out the picture tube faster if you set it higher.

As for how high is too high, you'll want to consult your owner's manual for safe operating conditions. Most of the time, it will only display refresh rates that your monitor is capable of, but if you're seeing refresh rates well in excess of 100 Hz, it's probably an indication that your computer isn't properly detecting your monitor.
